

About Our Service
The Coombe Hospital Pharmacy Department is registered with the Pharmaceutical Society of Ireland and operates under the rules outlined by the Pharmacy Act 2007, as amended.
The department currently consists of 6 pharmacists, 3.6 pharmacy technicians and 1 porter.
The pharmacy department provides a range of patient-centred and person-centred services to the hospital:
Procurement and supply of medicinal products.
Extemporaneous compounding.
Medicines information.
Antimicrobial stewardship.
Medicines Reconciliation.
Development and review of policies, procedures and guidelines related to the safe, effective and economic use of medicines (including electronic health record development).
Clinical pharmacy.
Education.
Attendance at hospital committees (e.g. guidelines, sepsis, infection prevention and control, postpartum haemorrhage, etc).
Coordination of the hospitals RSV prevention programme.
Development, maintenance and review of hospital prescribing guidelines and associated smartphone apps.
Coordination and management of parenteral nutrition supply.
Seamless discharge of neonatal patients requiring medication when going home.

Our Key Deliverables
Clinical Pharmacy
Clinical pharmacists work at both multi-disciplinary team (MDT) level and in a number of specialities including neonatal intensive care, microbiology, clinical trials and women with pre-existing conditions.
Clinical Pharmacists may participate in ward rounds and attend multidisciplinary team meetings where they offer their expertise in medicines use covering pharmacology, formulation science (pharmaceutics) and forensic pharmacy (i.e. ensuring medicines use occurs within rules outlined in legislation).
Clinical pharmacists visit patients at ward level and are involved in many aspects of patient care including drug usage review, promotion of safe, effective and economic medication use, patient and staff education, research, therapeutic drug monitoring, and other ward based clinical activities.
Drugs and Therapeutics Committee
The Hospital Drugs & Therapeutics Committee, chaired by the chief Clinical Operations Officer, provides overall governance of the medication use process in the hospital. A collaborative approach to medicines use governance between the pharmacy department and the DTC ensures all staff have access to evidence-based, up to date guidance on medications.
Medication Safety
The pharmacy department leads the hospital’s medication safety programme through the multidisciplinary Medication Safety Committee.
Pharmacy Technician Top-up service
The pharmacy provides a technician-led top-up service for all inpatient wards. This allows the pharmacy department to control and monitor medicines usage at a supply level while simultaneously freeing up nurse/midwife time for other patient-related activities. This person-centred approach has benefits for both staff and patients while fostering good communication and collaboration across disciplines.

There are a number of consultant obstetricians offering services through our private clinic. Antenatal care fees vary between consultants..

All appointments can be directed through Hospital reception: 01 408 5200, 24 hours a day -
Health Insurance:
We operate a direct payment scheme with VHI, Laya Healthcare, Irish Life Health, Garda Medical Aid, ESB, Medical Provident Fund, and Prison Officers Medical Aid. If you are covered by any of these insurers, the relevant insurance forms are signed electronically on your admission to hospital.
