Maternity Registration Form

Even if you've had a baby at the Coombe in the past, all maternity patients must fill in a Patient Registration Form. Please return the completed form to us in order to receive your first appointment. Click the word 'more' (below) to download a copy of the form.

Your outpatient appointment

Nearly 120,000 outpatient attendances take place in our clinics every year, and the number is growing. To help us give the best possible service to you we have set up a Central Appointments Office:

Patients, GPs and referring hospitals may now fax both Antenatal and Gynaecology referral letters and completed Patient Registration Forms to fax number 01 4085560. However, telephone calls will only be accepted on the following numbers:

For Antenatal enquiries only, please call 01 4085463 or 01 4085464.

To cancel or change an Antenatal appointment, please follow the advice further down this page.

For Gynaecology enquiries only,  please call 01 4085562

To cancel or change a Gynaecology appointment, please follow the advice further down this page.

If you can't keep your appointment

Please let us know as soon as possible if you can't keep your appointment, so that we can give your slot to another patient. You should telephone us, so that we can re-arrange another date for you. We have special telephone lines to take your message:

If you wish to change your ANTENATAL appointment, please telephone 01 4085483 and leave:

  • Your hospital number OR your name and date of birth
  • The Team you are attending (for example Team A, Dr O'Connell)
  • How many weeks pregnant you are now

Once we pick up this message, we will either telephone you back with a new appointment or send your new appointment to you by post.

If you wish to cancel, or change, your GYNAECOLOGY appointment, please telephone 01 4085495 and leave:

  • Your name and address
  • Your date of birth
  • The date of the appointment you wish to cancel or change
  • Whether you wish us to arrange a new appointment date for you

When you arrive

Please arrive in good time, taking into account that there is very limited parking on-site. The clinic you are due to attend will be listed on your appointment letter. If you are not sure where to go, please ask a member of staff. They will give you directions to the clinic or any other part of the hospital you are due to attend.

Please bring with you

  • Your appointment letter, or appointment card.
  • A list of any questions you might want to ask
  • Details of any medication you may be on. If possible, bring the medication with you.

Partners and family

We understand that you may want to bring a family member with you. Clinics can become very crowded so we do ask you to see your accompanying partner or family member does not take up a seat in the clinic forcing other patients, or pregnant mothers, to stand.

Children

Small children, particularly babies in buggies, should not be brought with you to your appointment unless your baby is being brought for review at a clinic.
